A stroke support group meets the second
Wednesday of the month from 1:30 to
3:30 p.m. The VNA Mobile Health Link Van
will be at RHCI the first Monday of the
month from noon to 4:30 p.m. for free
health screenings. The aphasia commu-
nity group meets the first Tuesday of the
month from 2 to 3:30 p.m. FoodAddicts in
RecoveryAnonymous meets Sundays from
9:30 to 11 a.m. The Well-Spouse Support
Group meets every third Thursday of the
month at 7 p.m. Healthy Hearts outpatient

The Visiting Nurse Association of
Cape Cod has a 28-foot van that travels
throughout the Cape to link underserved
individuals and families with health
education and preventative screening
services. Blood pressure, cholesterol,
blood sugar and more may be tested
at the van. The van will be at K-Mart
Plaza in Hyannis May 8 from 10 a.m. to
3 p.m. It will return May 10 when it will
be at Cape Cod Community College in
West Barnstable from 9:30 to 3:30 p.m.
Another unique service the VNA offers
is interpretation and tele-interpretation
in five languages: Portuguese, Span-
ish, Haitian Creole, Persian and Italian.

called Safe Return for those with

Bereavement groups
The Visiting Nurse Association of Cape
Cod Hospice Bereavement Program of-
fers a support group designed for men
who have lost a loved one. The group

Group for those who have lost children
meets Mondays at 1 p.m. at the VNA,

Parents Helping Parents, a self-help
support group, offers free weekly meet-
ings for parents to share understanding,

largest unified primary health care program
in the United States. In Massachusetts , more
than 185 practice sites serve nearly 700,000
Massachusetts residents yearly.
Community health centers, such as the
members of Cape & Vineyard Community
Health Center Network , can play a criticalrole
inmakingthis new Massachusetts experiment
a success. Health centers have a longstand-
ing commitment to, and expertise in, serving
low-income patients with complex medical
and social needs. Staffed by board-certified
physicians and a range of other medical and
social service providers, community health
centers excel at providing preventive care
and chronic disease management in lower-
cost community settings. These savings are
passed on to the state's Medicaid program
and other insurers.
Health centers were providing patient-cen-
tered care long before the concept became
industry "buzz."We have developed nationally
recognized programs of care for the chronically
illin diabetes,asthma, cardiovascular disease,
depression and cancer.
As multicultural organizations committed
to health promotion and disease prevention ,
members of the Cape & Vineyard Community
Health Center Network are typicalcommunity
health centers. We provide medical care and
social services in a patient-centered , cultur-
ally competent way. Staffed by caregivers
who speak the languages and understand the
cultures of their communities, health center
effectiveness isreflected by asignificant reduc-
tion in infant mortality and low birth weights,
and an increase in patient self-management
of chronic diseases such as diabetes. Sixty-
three percent of all health center patients
participating in a statewide model diabetes
program actively set goals to better control
their diabetes through diet , exercise and
improved monitoring of blood sugar levels.
Health center care generates savings for the
system as a result of treating patients who
would otherwise seek care through hospital
emergency rooms or end up in hospital beds
because of delayed treatment.
While there are still many details to work
through and implement, health centers are
eager to lend their community-based connec-
tions and expertise to make sure that everyone
in Massachusetts has comprehensive health
coverage they can both afford and use.
The central challenges in creating genuine
reform are affordability and accessibility of
health care. The potential premiums, deduct-
ibles and co-pays associatedwiththe new health
plans must be set at levels that acknowledge
the myriad financial burdens of low-income
individuals and families.
At the same time, having an insurance card
does not automatically provide a person with
access to health care. Health centers can play a
key role in educating patients about expanded
eligibility for the state's Medicaid program
and about the new insurance products that
will come through the Commonwealth Health
Insurance Connector - the authority that will
make it easier for uninsured residents to pur-
chase affordable insurance.
In addition to helping facilitate local health
access, health centers look forward to playing a
strongroleintrackinghealth disparitiesand im-
provinghealthoutcomes acrossMassachusetts
